The Rocky Mountain Multiple Sclerosis Center offers MS ID cards as a service to those who live with multiple sclerosis. The card lists vital information on one side and common MS symptoms on the back.

To receive an MS ID Card, please print, complete and return this PDF form.

Verification of your MS diagnosis is required along with a $10 processing fee. You may have your physician sign a statement of your diagnosis on medical office letterhead or a prescription pad. Physician’s verification may be faxed to 303-788-5418 or mailed to the address below.

Please note: MS ID Cards are processed once a month. Orders received after the 20th of the month may not be processed until the following month.

If you’re looking to replace a lost MS ID Card or update your information, please fill out the PDF form and send it in as instructed. You do NOT need to provide a physician’s confirmation if you’ve previously received a card from the Rocky Mountain MS Center.
